Home Health Aide (HHA)

Home Health Aide Essential Functions:

Provides direct client care at home following a specified care plan and adhering to safety protocols.
Requires current NYS HHA certification and a professional appearance.
Assists with personal care, household tasks, transportation, and medication reminders.
Monitors and reports on the client’s condition to the supervising nurse or case coordinator.
Offers emotional support and education to clients and their families.
Performs household support duties like linen changes, laundry, light cleaning, meal preparation, and engages clients in activities.
Involves physical activity including lifting, transferring patients, and working in varied home environments with potential exposure to patient elements.
