Home Health Aide Essential Functions:

Pre-Hire Questionnaire 

  • Provides direct client care at home following a specified care plan and adhering to safety protocols.

​

  • Requires current NYS HHA certification and a professional appearance.

​

  • Assists with personal care, household tasks, transportation, and medication reminders.

​

  • Monitors and reports on the client’s condition to the supervising nurse or case coordinator.

​

  • Offers emotional support and education to clients and their families.

​

  • Performs household support duties like linen changes, laundry, light cleaning, meal preparation, and engages clients in activities.

​

  • Involves physical activity including lifting, transferring patients, and working in varied home environments with potential exposure to patient elements.
