NHTD/TBI Waiver Programs​

The Traumatic Brain Injury (TBI) and the Nursing Home Transition and Diversion (NHTD) Medicaid Waiver programs offer specialized services for individuals with TBIs to address complex cognitive and physical challenges while living at home instead of nursing facilities, with home health aides providing essential daily support. These waivers offer comprehensive TBI support, including care coordination, home modifications, and therapies, to ease burdens on individuals and caregivers.

TBI Waiver requirements:

Serves adults 18+ with brain injuries that happened after age 18 (not from diseases that get worse over time). Your clients need Medicaid and must require the same level of care as in a nursing home.

NHTD Waiver requirements:

Helps adults with physical disabilities (ages 18-64) or seniors (65+). Your clients need Medicaid and must have documentation of their physical disability.

Through the BHRAGS NHTD/TBI Home Care Program, we help with:
  • Coordinating Waiver/non-standard Medicaid services

  • Arranging for Home and Community Support Services (HCSS)

  • Environmental & Vehicle Modifications (e.g., wheelchair-accessible bathrooms)

  • Skilled Nursing and/or Home Health Aides
